Does DBS work for head tremor? What the evidence shows
Yes, for most well-selected patients — but results depend on the cause of the tremor, the brain target and the surgical technique. Some patient-information pages state that DBS does not help head tremor [1]. The published evidence is more nuanced: in the largest clinical trial dataset of DBS for essential tremor, unilateral thalamic DBS significantly improved head and voice tremor, and staged bilateral DBS improved midline tremor further [2]; a 2025 single-center analysis of 54 patients found that 87% responded [3]. Dystonic head tremor is less predictable and depends heavily on the target [4]. In Prof. YILMAZ’s peer-reviewed series of 22 patients with rare tremor syndromes treated with double-target Vim+PSA DBS, the mean tremor reduction was 89% (range 50–100%), and 6 of 7 patients with isolated head tremor had a tremor score of 0 at 12 months, as rated blindly by two movement disorder specialists [5].
Key facts
- Larger and newer studies show meaningful improvement in head tremor with DBS [2],[3].
- Bilateral stimulation gives better midline control than one side [6].
- Dystonic head tremor is less predictable and target-dependent [4].
- Double-target series: mean tremor reduction 89% [5].
- Diagnosis, technique and programming matter as much as the target.
The main studies
| Study | Patients | Finding on head tremor |
|---|---|---|
| Mitchell et al., 2019 [2] | 119 unilateral; 39 staged bilateral (essential tremor) | Unilateral Vim DBS improved head tremor; bilateral added further midline improvement |
| Anis et al., 2025 (abstract) [3] | 54 (essential tremor with head tremor) | 87% responded; sustained over a median 14 months |
| Putzke et al., 2005 [6] | 22 (staged bilateral) | Bilateral stimulation added incremental midline improvement; head and voice most consistent |
| J Neurosurg 1999 [7] | 10 with severe head tremor | 9 of 10 improved markedly with bilateral thalamic DBS |
| Pauls et al., 2014 [8] | 7 (dystonia with head tremor), blinded | Head tremor amplitude improved 77.5% |
| 18-patient long-term series [9] | 18 (dystonic head tremor) | Vim DBS safe and effective with stable long-term results |
| Yilmaz et al., 2024 [5] | 22 rare tremors, double target | Mean reduction 89%; 6 of 7 isolated head tremor patients reached a score of 0 |
Why some sources are more cautious
- In older studies, head tremor was usually a secondary outcome; most operations were one-sided and aimed at the hand.
- Head tremor is a midline symptom and responds best when both sides are treated [6],[10].
- Dystonic head tremor behaves differently from essential tremor; with a single thalamic target, improvement in dystonic tremor has averaged 40–50% in pooled data [4].
- Reports on Vim-only DBS in rare tremors describe limited effects and frequent loss of benefit over time [5].
How our experience fits
Our published results and our experience since then point in the same direction: when the type of tremor is clearly identified, both sides are treated, and electrodes reach both the Vim and the PSA, head tremor can usually be stopped. The stimulator is switched on the day after surgery, so patients see the result before they leave. Our longest-followed head tremor patient still has a tremor score of 0 six and a half years after double-target DBS; the score was 4 before surgery (our case series; results vary). In our own case series of patients treated with DBS for head tremor, average improvement was 87%, and more than 70% of patients reached near-complete resolution (our case series; results vary).
What this means for you
DBS is a realistic option for disabling head tremor. The most important factors are an accurate diagnosis, an experienced surgical team, precise electrode placement and careful programming. This is the rationale behind the double-target technique.
